To better serve our customer's, we have created this simple form to request a service appointment. Please fill out the following information in order to better serve you. Please keep in mind, any service request over 1 year from the date of purchase will be assessed an $100.00 service fee. This fee includes one hour of touch up and adjustment at the original time scheduled only. Any additional time will be billed for accordingly at $65.00/hour. Also any materials or parts needed are at an additional charge. Thank you for your cooperation and business. You will be contacted within 72 hours to schedule an appointment.
